In plain words

In men whose prostate cancer has spread but still responds to hormone therapy, adding targeted radioactive treatment with lutetium-PSMA to standard hormone therapy delayed the growth of the disease on scans. Mild dry mouth was the most common side effect. Its effect on how long people live is not yet known.

Lutetium-177–PSMA-617 every 6 weeks; ADT (± ARPI) continuous
Duration

Per the FDA label and the trials. Patient selection by PSMA PET (centrally read in the trials). Only one 20% dose reduction, to 5.9 GBq (160 mCi), is allowed, and re-escalation is not permitted; if a second reduction is needed, discontinue; if a treatment delay due to an adverse reaction lasts more than 4 weeks, consider permanent discontinuation. Complete blood count and creatinine/creatinine clearance before and during treatment; withhold for creatinine clearance below 30 mL/min. The patient should drink plenty of water and urinate frequently before and after the infusion. Dry mouth is the most common adverse reaction. To be administered only by a physician licensed to use radiopharmaceuticals.

Flush the catheter before the injection with at least 10 mL of 0.9% sodium chloride. Only dose-reduction level: 20%, to 5.9 GBq (160 mCi); do not re-escalate; if a second reduction or a delay of more than 4 weeks is needed: discontinue. Grade 2 bone marrow suppression: withhold until grade 1; grade 3 or higher: withhold and reduce the dose; recurrence after reduction: discontinue. Renal toxicity (grade 2 creatinine increase or CLcr below 30): withhold until improvement; creatinine increase of 40% or more and CLcr decrease of more than 40%: withhold and reduce the dose; grade 3 or higher: discontinue. Grade 3 dry mouth or grade 3 or higher gastrointestinal toxicity refractory to treatment: withhold and reduce the dose. Monitor CBC and creatinine/CLcr before and during treatment; good hydration and frequent voiding before and after the injection; follow radiation protection principles; the partner should avoid pregnancy until 14 weeks after the last dose. In PSMAddition, the ADT + ARPI was chosen by the physician.

Doses follow the trial protocol, guideline or official drug label (source given in the notes of each regimen). Before prescribing, check blood counts, renal and hepatic function, drug interactions and the patient’s condition, and adjust the dose according to the drug label.

Treatment arms and regimens

1177Lu-PSMA-617 + ADT + ARPI (572 patients)

177Lu-PSMA-617 IV at 7.4 GBq (200 mCi) ±10% every 6 weeks for up to six cycles, with androgen-deprivation therapy (ADT) and an androgen receptor pathway inhibitor (ARPI) of the investigator's choice per local label.

2ADT + ARPI (572 patients)

ADT and an ARPI of the investigator's choice per local label; crossover to 177Lu-PSMA-617 allowed after centrally confirmed radiographic progression.

Source of the regimen details: Abstract of the published paper (Lancet 2026) and ClinicalTrials.gov NCT04720157

Doses are given for the information of specialists, as in this study’s protocol, and are not a prescription. Every drug must be prescribed by the treating physician for the individual patient.

Study population

Men with metastatic prostate cancer that was naive or sensitive to androgen pathway modulators (hormone-sensitive), untreated or minimally treated, with metastases on CT, MRI or bone scan and at least one PSMA-positive metastatic lesion on centrally read [68Ga]Ga-PSMA-11 PET. 50% de novo metastatic; 68% high-volume; median age 68 years.

Endpoints

Primary outcome: radiographic progression-free survival (central assessment by PCWG3-modified RECIST 1.1, or death). Secondary outcomes included safety and tolerability.

Safety and side effects

Grade 3 or worse adverse events occurred in 286 (51%) of 564 patients in the lutetium group and 243 (43%) of 565 patients in the control group. Serious adverse events were 32% versus 29%, of which 17 (3%) in the lutetium group were drug-related. The most common adverse event was dry mouth (46% versus 4%; all grade 1 or 2). Decreased blood counts and gastrointestinal disorders were more frequent in the lutetium group.

Clinical significance and place in guidelines

On 31 July 2026 the FDA approved 177Lu-PSMA-617 with an ARPI for PSMA-positive metastatic hormone-sensitive prostate cancer (mAPMN/S), extending the use of this radioligand from castration-resistant disease to earlier stages.

Limitations

Open-label design; interim analysis; immature overall survival; crossover of the control group to lutetium allowed; more grade 3 or higher adverse events.
