Anthracycline-free adjuvant treatment of breast cancer, every 21 days

Updated: 28 October 20261 min read1 reference

Summary for doctors

  • An anthracycline-free alternative in early breast cancer, especially when cardiac risk matters [1].
  • 4 cycles every 21 days [1].
  • In practice, febrile neutropenia is more common than in the original study; consider G-CSF prophylaxis.

Doses as in US Oncology 9735 [1].

Doses follow the trial protocol, guideline or official drug label (source given in the notes of each regimen). Before prescribing, check blood counts, renal and hepatic function, drug interactions and the patient’s condition, and adjust the dose according to the drug label.

Premedication and supportive care

  • Docetaxel premedication: dexamethasone 8 mg twice a day for 3 days starting the day before the infusion (for fluid retention and hypersensitivity).
  • Antiemetics: moderate emetogenicity (5-HT3 and dexamethasone).
  • G-CSF: many centres give primary prophylaxis, especially in older patients (G-CSF tool).

Monitoring and dose adjustment

  • Blood counts before each cycle; liver enzymes (docetaxel is more toxic with a raised bilirubin, or AST/ALT above 1.5 × with alkaline phosphatase above 2.5 ×).
  • Febrile neutropenia or prolonged neutropenia: docetaxel 60 to 65 mg/m², or G-CSF.

US Oncology 9735: after 7 years, TC gave better disease-free survival (81% vs 75%) and overall survival (87% vs 82%) than AC [1].
