Summary for doctors
- Residual invasive disease after neoadjuvant anti-HER2 treatment: 14 cycles [1].
- It roughly halved invasive recurrence (3-year invasive disease-free survival 88.3% vs 77.0%) [1].
- Do not substitute with trastuzumab or T-DXd; the doses are different.

| Drug | Dose | Route | Schedule |
|---|---|---|---|
| Trastuzumab emtansine | 3.6 mg/kg | IV (first dose over 90 minutes, then 30 minutes) | Day 1 |
Evidence: KATHERINE (2019)
Doses follow the trial protocol, guideline or official drug label (source given in the notes of each regimen). Before prescribing, check blood counts, renal and hepatic function, drug interactions and the patient’s condition, and adjust the dose according to the drug label.
Premedication and supportive care
- Antiemetics: low emetogenicity.
- Routine premedication is not needed; monitor for infusion reactions with the first dose.
Monitoring and dose adjustment
- Platelet count and liver enzymes before each dose; thrombocytopenia and raised enzymes are the most common reasons for dose reduction [1].
- Left ventricular ejection fraction (echo or MUGA) before starting and every 3 months during anti-HER2 treatment.
- Peripheral neuropathy and a new cough or dyspnoea (pneumonitis).
