Absolute neutrophil count equals white cells multiplied by the sum of segmented neutrophils and bands, divided by 100.

Definitions

  • Neutropenia: ANC below 500/µL, or below 1000/µL and expected to fall below 500/µL within 48 hours [1]. “Profound neutropenia” means below 100/µL [1].
  • Fever: a single oral temperature of 38.3 °C or higher, or 38.0 °C or higher sustained for one hour [1].
Febrile neutropenia is an emergency

Blood cultures (two sets, one from the catheter), baseline tests and broad-spectrum antibiotics within one hour of arrival [1]. Estimate the risk of complications with the MASCC score and, in clinically stable solid-tumour patients, with CISNE to decide on outpatient treatment [2].

Common questions

Should bands be included?

Yes: ANC = white cell count × (segmented neutrophils % + bands %) ÷ 100. Many laboratories report the ANC directly.
