The indices
| Index | Factors | Groups |
|---|---|---|
| IPI [1] | Age over 60, raised LDH, ECOG 2 or more, stage III–IV, more than one extranodal site | 0–1 low, 2 low-intermediate, 3 high-intermediate, 4–5 high |
| R-IPI [2] | The same five factors | 0 very good, 1–2 good, 3–5 poor |
| NCCN-IPI [3] | Age (41–60: 1, 61–75: 2, over 75: 3), LDH ratio (over 1 to 3: 1, over 3: 2), stage III–IV, involvement of bone marrow/CNS/liver-GI tract/lung, ECOG 2 or more | 0–1 low, 2–3 low-intermediate, 4–5 high-intermediate, 6–8 high |
In patients treated with R-CHOP, NCCN-IPI separated the high-risk group better than the IPI: 5-year survival about 96% in the low-risk group versus about 33% in the high-risk group [3].
The indices do not change first-line treatment (R-CHOP or Pola-R-CHP) but help counselling and trial selection. CNS-IPI (the same five factors plus kidney or adrenal involvement) estimates the risk of CNS relapse: at 2 years about 0.6% in the low group (0–1), 3.4% in the intermediate group (2–3) and 10.2% in the high group (4–6) [4]. Regimens: R-CHOP, Pola-R-CHP.
