Resectable gastric or GEJ cancer; before and after surgery
| Drug | Dose | Route | Schedule |
|---|---|---|---|
| Docetaxel | 50 mg/m² | IV over 1 hour | Day 1 |
| Oxaliplatin | 85 mg/m² | IV over 2 hours | Day 1 |
| Leucovorin | 200 mg/m² | IV | Day 1 |
| Fluorouracil | 2600 mg/m² | Continuous 24-hour infusion | Day 1 |
Doses as in the FLOT4 study [1]. Adding durvalumab to FLOT has been studied in the MATTERHORN trial (news).
Evidence: FLOT4 (2019)
Doses follow the trial protocol, guideline or official drug label (source given in the notes of each regimen). Before prescribing, check blood counts, renal and hepatic function, drug interactions and the patient’s condition, and adjust the dose according to the drug label.
Premedication and supportive care
- Docetaxel premedication: dexamethasone (per local protocol).
- Antiemetics: 5-HT3 and dexamethasone.
- Consider G-CSF prophylaxis (G-CSF tool).
- DPYD: before the first fluoropyrimidine dose, consider testing for DPD deficiency (DPYD genotype); in carriers of one reduced-function allele the starting dose is reduced by about 50%, and these drugs are contraindicated in complete deficiency [2].
Monitoring and dose adjustment
- Full blood count before each cycle; common thresholds ANC 1500 and platelets 100,000.
- Neuropathy and fluid retention (docetaxel); diarrhoea and mucositis (fluorouracil).
- After surgery, many patients do not complete the postoperative cycles; optimise nutrition before starting.
