CLL-IPI was derived in the chemoimmunotherapy era [1]. Today its most important part is testing TP53 (17p deletion and mutation) and IGHV before any treatment, because they decide the treatment: with TP53 aberration or unmutated IGHV, chemoimmunotherapy (such as FCR) has no place and targeted therapy (a BTK inhibitor or venetoclax-obinutuzumab) is the choice. Starting treatment is still based on the iwCLL criteria, not the score [2].
